Cô sinh viên 20 tuổi chưa có chồng nhưng ngực liên tục tiết sữa non, trong khi kinh nguyệt thưa dần rồi mất hẳn.

N.H.Y là sinh viên trường Cao đẳng Nghệ thuật tại Hà Nội. Cô gái trẻ có kinh nguyệt từ năm 14 tuổi, nhưng đến năm 17 tuổi đột nhiên chu kỳ thưa dần rồi mất hẳn.

Y. chủ quan không đi khám. Cho đến 1 năm trở lại đây, cơ thể xuất hiện thêm nhiều dấu hiệu bất thường như ngực liên tục tiết sữa, cứ nặn là ra sữa non, da toàn thân khô sạm dần, hay đau đầu, cô gái trẻ mới đi khám phụ khoa vì lo vô sinh.

Tuy nhiên cả tử cung và phần phụ đều hoàn toàn bình thường. Khi đến khoa Ngoại thần kinh, BV K làm xét nghiệm, các bác sĩ phát hiện nồng độ hormone tuyến yên trong máu tăng cao bất thường.

Phụ nữ tiết sữa bất bình thường, mất kinh nguyệt đột ngột, da thô ráp là những dấu hiệu chính giúp bạn nghĩ đến u tuyến yên

Khai thác thêm, bệnh nhân cho biết khi lượng kinh nguyệt giảm càng nhiều, lượng sữa tiết ra càng lớn.

Ths.BS Nguyễn Đức Liên, Trưởng khoa Ngoại thần kinh cho biết, trường hợp này bệnh nhân đã hình thành u tuyến yên.

“Nhưng do khối u còn nhỏ nên bác sĩ chỉ định dùng thuốc để ức chế tế bào u. Sau vài tháng điều trị, bệnh nhân đã có kinh nguyệt trở lại, da dẻ mịn màng hơn”, BS Liên chia sẻ.

Bệnh nhân được chỉ định tiếp tục dùng thuốc duy trì, tái khám theo lịch hẹn, tiên lượng rất khả quan, sau này có thể có con bình thường.

BS Liên cho biết, đã từng khám và điều trị cho rất nhiều trường hợp phụ nữ mắc u tuyến yên mà không biết. Hầu hết đều có hiện tượng tiết sữa bất thường và đều tìm đến các chuyên khoa phụ sản để thăm khám.

Trong đó có nhiều trường hợp đã cai sữa 3-4 năm vẫn có hiện tượng tiết sữa, nhưng chủ quan, chỉ phát hiện bệnh khi tình cờ đi khám bệnh lý khác.

Theo BS Liên, tuyến yên là tuyến hình hạt đậu nhỏ nằm ở đáy não, hơi phía sau mũi và giữa hai tai. Mặc dù kích thước nhỏ nhưng tuyến yên có nhiệm vụ điều hòa sự cân bằng các nội tiết tố (hormone) của cơ thể.

U tuyến yên có thể xảy ra ở cả 2 giới với mọi độ tuổi, tuổi càng cao, nguy cơ càng lớn. Khối u này khiến tuyến yên tạo ra quá nhiều hoặc quá ít hormone, ảnh hưởng đến các chức năng trong cơ thể hoặc gây áp lực lên não gây đau đầu cùng các triệu chứng khác.

Với u tuyến yên, tuỳ thuộc vào kích thước, khối u dạng không chế tiết hay chế tiết loại hormone nào mà bác sĩ sẽ đưa ra phương pháp điều trị khác nhau.

Với trường hợp u tuyến yên chế tiết Prolactin khiến phụ nữ tiết sữa dù không mang thai như trường hợp N.H.Y không nhất thiết phải mổ nếu khối u nhỏ và không giảm thị lực.

Các trường hợp phải phẫu thuật đa số được mổ nội soi qua mũi, xoang, đảm bảo thẩm mỹ. Những khối u lớn, có thể phải mở hộp sọ.

BS Liên khuyến cáo, phụ nữ khi có những dấu hiệu bất thường như đột ngột mất kinh, có hiện tượng tiết sữa dù không nuôi con nhỏ hoặc cai sữa đã lâu, thỉnh thoảng đau đầu… cần kiểm tra tuyến vú và kiểm tra nội tiết tuyến yên. Ở hầu hết những phụ nữ mắc u tuyến yên, da sẽ thô ráp, sần sùi hơn bình thường.

Giảm thị lực không rõ nguyên nhân cũng là dấu hiệu cần lưu ý vì có thể khối u lớn chèn ép dây thần kinh.

“Chúng tôi từng khám cho bệnh nhân giảm thị lực tới 5 năm nhưng không tìm ra nguyên do, khi kiểm tra phát hiện u tuyến yên to tới 5cm”, BS Liên chia sẻ.

Với nam giới, giảm ham muốn tình dục là một trong những dấu hiệu cần lưu ý hoặc những trường hợp trẻ em dậy thì quá sớm.
